CRANE is a social ecosystem (platform) for interaction between patients with heart failure, doctors, healthcare and social service providers, and patient care programs. The project is being implemented in accordance with the European concept of the European Health Data Space. The goal of the project is to integrate Cardiolyse AI Engine, Garden of care, and existing digital applications for doctors into a single ecosystem for health, prevention, and treatment of cardiovascular diseases. The ecosystem will be tailored to each individual's requirements to improve control, safety, security, freedom, and awareness of citizens' well-being.
